Had disconnected thoughts (he would see a car engine, then a fridge, then a book) He would get very down and not want to leave his place during the day (even thought it was so nice outside)

looks at such conditions (bipolar disorder, BPD,NPD, etc) as significantly poor negative affect regulation or emotion dysregulation disorders. Actually, that can apply to several mental conditions.
That is why there is a high comorbidity of conditions like ADHD with narcs..... ADHD is cognitive dysregulation.... essentially dysregulation of the attention system.
They do seem to have a higher incidence of other conditions in addition to narcisssism (i.e., ADHD, etc)... they are wired a bit differently and their brains react differently to stimuli. This has been proven via several neuroimaging studies. Therefore they will be prone to emotional and cognitive regulation problems that appear to lie outside of NPD.
That emotional dim-wit creature of a narc I was with was NPD and ADHD-hyperactive type. The ADHD was certainly no biggie to me at all... it was that darn soul crushing NPD that ripped my world apart.
One of my FAVORITE books is written by Allan Schore entitled "Affect Regulation and the Origin of the Self". It is a bit of a heavy read (may require some knowledge of neuroanatomy and neuroscience).... however he explains conditions such as NPD, BPD (borderline PD), bipolar disorder, etc, from the standpoint of neuroanatomy. He blends neuroscience and psychology.... which is right up my alley!! I just LOVE that book!

On a more serious note, my STBXNH's sister was bi-polar. She eventually committed suicide after years of trying. She also was convicted of felony arson. Some valid reasons there for concern about his genetics. STBX is dyslexic and probably would have been diagnosed ADHD if that had been done more when he was a kid. He is also prone to depression, but he masks it with false exuberance. Kind of like Teddy Roosevelt was/did. STBX does not do the "depression" side of the mania in the typical form, but I think his need to sleep that borders on having narcoleptic urgency might be his way of checking out when he has run out of things to be ecstatic about and/or some sort of reality is trying to make its way into his brain.

My Narc would cry after phone sex (he did all the talking too and would get mad if I didnt seem into it)
He would then act like he was having chest pains and had to get off the phone. Found out he did the same with the OW acted all odd after.
Now, when i took him back when he begged me...he didnt even try..he said he was disgusted with himself and would never do that to another woman again.
For days after the phone sex he swore we would fight because God was punishing us.

Twisted indeed!!! I never understood the whole fearing God thing. He told me he has always struggled with intimacy. I think something must have happened way back when he was younger....or maybe the whole catholic guilt thing...i dunno.
Crying after phone sex though is just odd....I think he only did that with me bc the California OW (see mine had a cali woman too) the greater the distance the safer it was I guess. she didnt say he cried after to her.
