How do you get a website ? OT

Lets say i know what website name i want and when i do a search for it, nothing comes up, nobody owns it, not even those companies that keep domain names. What do i do next?

Lets also say i know what name i want and when i do it search it is owned by one of those domain name distributers and it says I have to pay 60 bucks... Does this mean 60 bucks per month or 60 bucks and its all mine?
